REQUEST FOR A PRIEST FOR PUJA
AT THE DEVOTEES' RESIDENCE

1. The undersigned request DURGA MANDIR to send a priest at the address given below for puja at the residence of_________________________on______________(date). The nature of puja is_____________________________.
2. The undersigned agrees to make arrangements to pick up the priest from the DURGA MANDIR premises at___________(time). If the priest is not picked up within one hour of the stated time, DURGA MANDIR reserves the right not to send the priest to the devotee's residence on the requested day. In that case, the devotee will have to make another request for some other day.
3. The undersigned agrees to make arrangements to bring the priest back to DURGA MANDIR at__________(time). The undersigned understands that the priest can not be away from the MANDIR for a period of more than 3 hours.
4. The undersigned agrees to make donation (minumum donation has been suggested in our schedule of special services) for maintenance of the MANDIR. Priest's "DAKSHINA" will be paid directly to the priest by the devotees.

(Note: It is important that the timings for picking up the Priest from the MANDIR and bringing him back to the MANDIR are strictly observed because the priest may be scheduled to visit other devotees at their residence for puja or his services are needed at the MANDIR.)
